The coroner on occasions needs to be contacted urgently, out of hours (for instance in the context of requesting permission to proceed with organ donation).  The Worcestershire coroner holds a mobile phone, and can be called out of hours for advice.  His number is written in the ICU doctors office if you do not have it.

The form also asks for an email address – this needs to be the bereavement office email not the referrers so all communication from the coroner comes via the bereavement office not directly to the referrer.
