Information on Equipment Commonly Used on the Critical Care Units

VENTILATORS

Worcestershire uses Evita (v500, v800) and Savina ICU ventilators on ICU, along with Oxylog 3000 transport ventilators in ED and ICU. Anaesthetic machines have advanced ventilators (Maquet Flowi)

Hamilton T1 transfer ventilators
(on transfer trolleys)

Hamilton T1 ventilators are located on both ICU transfer trolleys, but can be detatched and used as a stand alone transfer ventilator.  It has some advantages over the oxylog transfer ventilator, in particular with smaller children.

Worcester ICU has access to a Novalung extra-corporeal machine for the advanced management of respiratory failure to enhance C02 clearance.

AnaConDa Bronchodilator

AnaConDa device, with gas absorbing system, can be used for treatment of severe-bronchospasm (with additional general anaesthetic properties).  As scavenging is not available on the Alex site, Anaesthetic machines cannot be used on the unit, and so the AnaConDa system is preferred.  Either an Anaesthetic machine or the AnaConDa could be used on the Worcester site.

Section 1 deals with rapid set up of the system in the emergency setting.

​

Section 2 includes setting up and installing the gas analysis, which can be done either at initial setup, or once the patient has been stabilised if user not fully familiar with system.

Aeroneb - Nebulised Prostacyclin

Nebulised Prostacyclin (nPGI2) can be used for pulmonary dilatation in the management of right heart strain and hypoxia in the context of pulmonary hypertension.

​

Use with viral filters on the ventilator can be problematic because of the oily nature of the drug, which necessitates extra vigilance.

Rotem

A Rotem machine is available on the WRH site in main theatres recovery (next to ABG machine).  Alistair Mcminn leads on training for this machine.  

Please fill one of the light blue (citrated) tubes stored next to the machine (slightly larger than standard tubes), ensuring filled to the line with blood.

ULTRASOUND

The ICUs in Worcester have a number of both handheld ultrasounds for echo/vascular access as well as the larger ultrasound machines on wheels.

Handheld Ultrasounds for vascular access

The WRH ICU has a portable hand held Philips lumify ultrasound probe suitable for echocardiography. It has a tablet with with the app installed

Both WRH and Alex ICUs have portable vascular access ultrasounds, which can pair with your mobile phone and be used around ICU/the hospital to aid in vascular access

LIDCO Cardiac Output Monitors

Both Intensive Care Units, and theatre complexes at Worcester and Redditch have the Lidco cardiac output monitor available to use.

PA catheters: Hemisphere Cardiac Output Monitor

In addition to Lidco, The Worcester site has cardiac output monitoring using the Edwards Lifesciences HEMISPHERE

EMERGENCY TRANSVENOUS PACING

Transvenous Pacing
  • Both Intensive Care Units carry a small stock of emergency transvenous pacing wires, which can be inserted by floatation without access to screening.

  • Temporary pacing boxes can be located in Alex or WRH ICU

  • The following presentation by Steve Haynes summarises the insertion and setup of a pacing wire in the emergency setting.

  • If time and patient stability allows, transvenous pacing is ideally performed by cardiologist under screening
